Rapid Medical Response of Homonymous Hemianopsia after Occipital Cortex Ischemia

Dr. Rapid recovery of right homonymous hemianopsia (HH) with medical treatment in a case of left occipital cortex ischemia is described. A 48-year-old white male with a history of cardiac bypass surgery, hypercholesteremia, and heavy smoking had calcified atheroma plaques located in the bilateral carotid arteries. Left occipital cortex ischemia developed after an embolic attack to the left posterior cerebral artery from the internal carotid artery. Intravenous high-dose methylprednisolone associated with hyperbaric oxygen, antiaggregant, anticoagulant, antioxidant, and ocular hypotensive therapy was initiated within 24 hours of the incident. Rapid recovery of right HH was observed within a week of treatment. This case illustrates the beneficial effect of high dose intravenous steroid associated with hyperbaric oxygen, antiaggregant, anticoagulant, antioxidant, and ocular hypotensive therapy for HH.
