Development of Intracranial Abscess and Choroidal Tuberculoma in A Case with Chronic Renal Failure

4Başkent Üniversitesi Hastanesi, Göz Hastalıkları Anabilim Dalı, Ankara, Prof. Dr. A patient with choronic renal failure (CRF) who has intracranial abscesses and choroidal tuberculoma and the development of optic neuropathy due to antituberculous therapy is presented in this case. A thirty year-old woman with CRF applied to our hospital with headache, mental confusion and blurred vision. In the cranial magnetic resonance imaging (MRI), focuses of multiple abscesses were detected. In the fundus examination, solitary, 1.5 disc-diameters, elevated choroidal tuberculoma at the infero-temporal area in his right eye was detected. Antituberculosis therapy started to the patient who had tuberculosis detected with laboratory tests. The tuberculoma regressed to scar tissue with the therapy almost in one month. After the 3rd month of the treatment, optic neuropathy due to ethambutol was developed and the drug protocol was changed. The minimal improvement was detected in the visual field defect during the follow-up.
Tuberculosis can make involvement in the brain and choroid in patients with CRF. During the anti-tuberculous therapy, patients should be followed carefully and the possibility of the development of the optic neuropathy should be considered.
