Intravitreal Dexamethasone Implantation in the Treatment of Recalcitrant Diabetic Macular Edema with Epiretinal Membrane

In our country, intravitreal dexamethasone implant is paid as off-label for the treatment of diabetic macular edema. A 69-year-old female patient who got various treatments for the management of diabetic macular edema and had recalcitrant diabetic macular edema and epiretinal membrane (ERM) in both eyes was planned to perform intravitreal dexamethasone implantation. We performed intravitreal injection to her left eye that had worse clinical findings with recalcitrant macular edema. There was improvement in visual acuity as well as decrease in macular edema and regression of macular traction due to partial seperation of ERM from the surface of retina for the first two months. In the third month it was observed that macular edema and ERM were began to increase again. In patients with macular edema and concomittant vitreomacular traction dexamethasone intravitreal implant decreases edema and seperates ERM from retinal surface. However, it should be considered that seperation of ERM after intravitreal dexamethasone implant may cause tractional complications.
