EXPERIMENTAL PROPHYLAXIS OF STAPHYLOCOCUS AUREUS ENDOPHTHALMITIS AFTER VITRECTOMY: CLINICAL AND HISTOPATHOLOGIC EVALUATION
1Fırat Üniversitesi Tıp Fak. Göz Hast A.D. Elazığ2Fırat Üniversitesi Tıp Fak. Patoloji A.D. Elazığ Purpose: The aim of this study was to investigate the prophylactic efficacy of vancomycin in irrigating solution during vitrectomy to prevent Staphylococus aureus (S.aureus) endophthalmitis.
Materials and Methods: This study was carried out on one eye of 36 healthy pigmented rabbits each weighing 2000-3000 g. The rabbits were divided into six groups each containing six rabbits. Vitrectomy was performed to all of the rabbits. Vitrectomy using 100 ml of BSS (balanced salt solution) irrigating solution with or without vancomycin (20µ/ml) was followed by intravitreal injection of S. aureus. In Group 1, BSS was used , followed by injection of 1.000 colony forming units (CPU) S. aureus; In Group 2, BSS plus vancomycin (VBSS) was used, followed by intravitreal injection of 1.000 CPU S. aureus; In Group 3, BSS was used followed by injection of 2.000 CPU S. aureus; In Group 4, VBSS was used followed by injection of 2.000 CPU 5. aureus; In Group 5, BSS was used followed by injection of 4.000 CPU S. aureus; In Group 6. VBSS was used followed by injection of 4.000 CPU S. aureus. The eyes were examined clinically for 5 days after surgery. At the end of fifth day, histopathologic examination was performed.
Results: Endophthalmitis developed in all eyes in groups 1, 3, 5 (not given vankomycin) and Group 6 (was given vancomycin). Endophthalmitis did not developed in groups 2 and 4. The concentration of 20 µ/ml vancomycin significantly diminished the intraocular inflammation after injection of 1000 and 2000 CPU S.aureus(p<0.01). Vancomycin in the irrigating solution at this concentration did not significantly effect incidence of endophthalmitis when 4.000 CPU S. aureus was injected (p>0.05).
Conclusion: In this animal model, vancomycin in a concentraiton of (20µ/ml) appears to prevent postvitrectomy S.aureus endophthalmitis when given in the infusion solution. Keywords : Endophthalmitis, Vancomycin, Vitrectomy, Prophylaxis
