PRIMARY VITRECTOMY IN POST-CATARACT RETINAL DETACHMENTS

Purpose: To evaluate primary pars plana vitrectomy (PPV) in cases of retinal detachment following catarakc extraction. Metarials and Methods: In our clinic 29 eyes of 29 patients where operated for retinal detachment after cataract surgery. The mean age of the patients undergoing operation (PPV) was 53 years (27 - 78). Eight of these (27.5 %) where female and 21 (72.4 %) where male patients. Mean follow-up time was 9.3 (1- 30) months.
Results: Of patients operated for retinal detachment after cataract extraction, 23 (79.3 %) had undergone planed extracapsular cataract extraction + IOL implantation, 3 (10.3 %) had undergone intracapsular cataract extraction and remaining 3(10.3 %) patients had phacoemulsification. Prepperatively 16 (55.1%) of patient had total retinal detachment with undetected tear, 23 (79.3%) had progressive PVR . After PPV operation attached retina was achived in 75.8% (22) patients. 10.3% (3) patient had intraocular bleeding and in 68.9% (20) patients had intra ocular high pressure problems.
Conclusion: Retinal detachment after cataract extraction operation differs somewhat from phakic retinal detachment mainly for place of tears which are situated more peripherally. Once detachment has occurred there are stronger chances for total retinal detachment, PVR and macular involvement. All these mentioned factors with difficulties in visualisation of retina make post operative prognosis more unfavorable. Keywords : Cataract extraction, retinal detachment, pars plana vitrectomy
