Pars Plana Filtration Combined With Vitrectomy For Refractory Glaucoma

Seventeen eyes of 15 patients with different forms of refractory glaucoma (13 neovascular, 3 aphakic, 1 sekondery to uveitis) underwent pars plana filtration surgery combined with vitrectomy. While in eight eyes, intra ocular pressure (IOP) was 80-70 mmHg. without medical therapy and 60-50 mmHg. with medical treatment, in nine eyes, IOP was 70-80 mmHg. without medical therapy and 50-45 mmHg. with medical therapy preoperatively, 6 months after the operation, in 35% of cases (6 eyes) IOP has been 21 mmHg. or less with only surgery, in 59% (10 eyes) was 21 mmHg. 21 mmHg. or less with surgery + beta bloker, 6% (1 eye) was 21 mmHg. with surgery +beta bloker + intra vitreal tube implantation. While in 59% of cases (10 eyes) visual acuity was between hand movement (HM) and 4/10 preoperatively, in 88% (15 eyes) was found between HM and 6/10 in the follow-up period. Complications included anterior chamber hemorrhage in two eyes (12%), fibrinoid reactions in 3 eyes (17%), flat anterior chamber in one eye. Pars plana filtration surgery combined with vitrectomy is an efficacious method for refractory glaucoma but this procedure should combine with intravitreal tube implantation in glaucoma secondary to uveitis.
