Review of Cases with Intraocular Lens and Nucleus Drop into the Vitreous Cavity

3Celal Bayar Üniversitesi Tıp Fakültesi Göz Hastalıkları A.D., Manisa, Arş. Gör. Dr. Purpose: To evaluate the results of pars plana vitrectomy (PPV) in the management of intraocular lens (IOL) and nucleus drop cases during or after cataract surgery.
Materials and Methods: Seventeen patients treated in Celal Bayar University Department of Ophtalmology for nucleus and IOL drop into the vitreus cavity between November 2005 and June 2008 were included in this study. There were five cases of IOL drop, eleven cases of nucleus drop and one case of both IOL and nucleus drop into the vitreus cavity. All patients underwent three port 20G pars plana vitrectomy. Afterwards in 13 patients IOL or nucleus were floated with perflourocarbon and removed from limbal incision. Five patients were treated with phacofragmentation. Etiology , applied surgical technique, preoperative and postoperative complications , preoperative and postoperative visual acuity were investigated from records.
Results: Ten patients were female and seven were male with a mean age of 69.8±30.4 years. Mean follow-up time for the patients was 4.3±2.4 months (2-10 months). Reason for dislocation was trauma in three patients while dislocation ocurred intraoperatively in two patients but reasons for dislocation were unknown in other patients. The most common complication was corneal oedema both in the preoperative and in the postoperative period. The preoperative visual acuity was P(+)P(+) in 7 patients (%41.2), HM in 7 patients (%41.2) and counting fingers at 1 meter in 3 patients (%17.6). Four patients left aphacic, sutured IOL was implanted to five patients, two patient’s IOL was implanted in ciliary sulcus and anterior chamber IOL was implanted in three patients. One patient’s dislocated IOL was sutured to sclera from haptics. Two patients with nucleus drop had their IOLs left in same position because of their good centralization. The final visual acuity was HM in 2 patients (%11.8), counting fingers at 1 meter in 2 patients (%11.8), 0.05-0.5 in 10 patients (%58.8) and 0.6 or beter in 3 patients (%17.6). In none of the cases uveitis, persistant corneal edema, retinal detachment were observed.
Conclusıon: Successful visual and anatomical results can be obtained from nucleus and IOL drop into vitreus cavity by pars plana vitrectomy techniques . Keywords : IOL dislocation, nucleus dislocation, pars plana vitrectomy
