Outcomes of Primary Pars Plana Vitrectomy in Patients with Rhegmatogen and Tractional Retinal Detachment

Dr Purpose: To assess the clinical outcomes of our patients with rhegmatogen and tractional retinal detachment who primarily underwent pars plana vitrectomy (PPV).
Materials and Methods: The results from 48 eyes of 47 patients with tractional retinal detachment (TRD) and rhegmatogen retinal detachment (RRD) (age range 11-79, female/male: 23/24, 48.9%/51.0%) between December 2005 and March 2008 were investigated retrospectively. Preoperative and postoperative ocular findings, visual acuity, and peroperative and postoperative complications were documented in detail. In addition, lensectomy was applied in some cases when necessary. All operations were performed with a standard three-port PPV setting. Core and peripheral vitrectomy, dissection and excision of membranes, removal of tractions, retinotomy and retinectomy, liquid and gas perflorocarbone, intravitreal triamcinolone injection, silicone oil, endolaser, and scleral buckling procedures were applied.
Results: There were dense vitreous hemorrhages in 23 eyes (47.9%), TRD in 20 eyes (41.6%), tractional-regmatogenous retinal detachment in 8 eyes (16.6%), RRD in 8 eyes (16.6%), and severe proliferative vitreoretinopathy in 2 eyes (7.4%). Retinal reattachment was achieved in 46 eyes (95.8%) with retinal detachment. Visual acuity improved in 13 eyes (50%), remained the same in 8 eyes (30.7%), and decreased in 5 eyes (19.3%).
Conclusion: Primary PPV is a favorable procedure for patients with RRD, TRD, or both, and affects the anatomical and functional prognosis depending on the first examination time, visual acuity, and regular follow-up.
