Management of Patients with Endophthalmitis, Methods and Results

Material and Methods: Among 196 patients who had diagnosis of endophtalmitis in Erciyes University Medical Faculty Ophthalmology Clinic January 2004 to January 2011, 42 patients whose data were obtained and at least one month follow up findings were included in retrospective study. The vitreal sampling and intravitreal injection of vancomycin+ceftazidime+dexamethasone were performed within the first 24 hours. Systemic and topical fortified antibiotics with topical steroid therapy were applied. Pars plana vitrectomy was performed on patients who had no response to medical treatment.
Results: The average age of patients was 61±14, the median value of follow-up period was 2.5 (1-42) months. The endophthalmitis was developed secondary to cataract surgery in 34 patients, intravitreal injection in 3 cases, trauma in 3 cases, trabeculectomy in 1 case, pars plana vitrectomy in 1 case. Pars plana vitrectomy was performed on 4 patients who were resistant to medical treatment. The best-corrected visual acuity (BCVA) median values of the patients were 2.6 (0.3-2.6) before treatment and 1.3 (0-2.6) in the last control. In 18 cases (42.8%), BCVA was 0.1 and on. 6 (14.2%) cases showed positive cultures. Despite all the treatments in 2 patients required evisceration. Despite treatment, 2 patients lost their sense of light.
Conclusion: Prompt diagnosis and treatment are important factors for determining prognosis in patients with endophthalmitis. Keywords : Endophthalmitis, intravitreal injection, pars plana vitrectomy
