Surgical Treatment in Diabetic Macular Edema

İmpairment of inner and outer blood-retina barriers in diabetes
results in macular edema and subsequent visual loss. Regulation
of systemic risk factors is important in the treatment of diabetic
macular edema (DME). In addition, macular laser photocoagulation,
intravitreal corticostreoids and frequently intravitreal
anti vascular endothelial growth factors are often performed for
the treatment of DME. Some patients have vitreomacular interface
abnormalities such as vitreomacular traction or epiretinal
membrane. In eyes with DME and vitreomacular traction or epiretinal
membran, successful results can be achieved with pars
plana virectomy (PPV) ± internal limiting membrane (ILM) removal.
However, PPV ± ILM removal is contraversial in eyes with
DME and without vitreomacular interface abnormalities. Prospective
randomized comparative studies are needed for a reliable
conclusion.
