THE MANAGEMENT OF A CASE OF RETINOBLASTOMA WITH ORBITAL INVOLVEMENT

4Dokuz Eylül Üniv.Tıp Fak. Patoloji Anabilim Dalı. A 2 - 5 year - old girl presented with a history of leucocoria, progressive proptosis and periorbital inflamation of her left eye for 5 months. Computerized tomograms and magnetic resonance imaging demonstrated a large calcified left eye with markedly enlarged optic nerve and lateral rectus muscle invasion of the tumor. Based on the needle biopsy reports the diagnosis of retinoblastoma was strongly suggested.
The patient was treated with combined chemotherapy (vincristine, cyclophosphamide, methotrexate) and radiotherapy which resulted in marked response. After normalization of the optic nerve and orbital tissues, the eye was enucleated and a silicon sphere was implanted into the orbit. The histopathologic examination revealed no tumor cells in excised conjunctiva, muscle tissues and optic nerve and the tumor inside the glob was necrotic.
The importance of preoperative chemotherapy and radiotherapy in the treatment of delayed retinoblastoma with orbital invasion was emphasized. Keywords : Retinoblastoma, chemotherapy, radiotherapy, enucleation
