Evaluating the Efficacy of Combined Photodynamic Thrapy with Intravitreal Bevacizumab Injection for Choroidal Neovascularization Arising from Different Etiologies

Objective: To compare the effect of combined photodynamic therapy with intravitreal bevacizumab injections for age related choroidal neovascularization (CNV) versus non-age related CNV.

Methods: This retrospective clinical trial enrolled 32 eyes of 31 patients with CNV. Group 1 consisted of 22 eyes with age related CNV. Group 2 involved 10 eyes with non- age related CNV. All patients were treated with combined photodynamic therapy with intravitreal bevacizumab injections. Patient demographics were analyzed, intra and intergroup statistic evaluations were done from the point of number of the injections, best corrected visual acuities (BCVA) and intraocular pressures (IOP).

Results: There was no significant difference for the mean number of injections between two groups (p=0.52). In both groups BCVA improvement was significant (group 1; p = 0.01, group 2; p = 0.04) after treatment. There was no significant difference with regards to maximum BCVA and letter gain between groups (p=0.48, p=0.20 respectively ). Mean IOP measurements did not significantly change (group 1; p=0.07, group 2; p=0.32) after treatment in both groups. There were no significant difference in the mean IOP values between groups (p=0.86).

Conclusion: To the best of our knowledge there is no study comparing the combined therapy between age related CNV and non-age related CNV in the literature. Combined photodynamic therapy with intravitreal bevacizumab injections has statistically significant effect on BCVA improvement devoid of raise in IOP. In our study there is no difference between age related and non-age related CNV from the point of BCVA improvement and the number injections.
